For Employer-Mandated Tests:
If your visit (such as a drug test or pre-employment physical) is paid for and required by your employer, the results will be securely transmitted directly to your company’s designated representative. Our staff cannot release these specific results directly to you at the clinic.
For Personal / Urgent Care Visits
If you are visiting for personal urgent care or travel vaccinations, your records are completely private and will only be shared with you or your primary care physician upon your written request.
🪪 Valid Photo ID: A current, unexpired government-issued ID (such as a Driver’s License, State ID, or Passport). Note: We cannot perform any testing without proper identification.
📄 Employer Authorization Form: If your employer sent you, please bring the authorization form or chain-of-custody paperwork they provided.
💊 Medication List: A list of any prescription or over-the-counter medications you are currently taking, especially if you are here for a physical exam or drug screening.

Q: What should I expect during a drug screen?
A: You will be asked to provide a sample (urine, hair, or saliva) in a secure, private, and controlled environment. You may be asked to empty your pockets and leave personal belongings in a secure locker during the collection process to ensure the integrity of the test.
Q: Do you accept health insurance?
